We want to hear from you!

Complete your hospital evaluation form after completing an assignment.

Your submission will help provide other travel nurses a better look at hospitals around the country. The sole purpose of this is not to degrade a hospital, but for travel nurses to express their honest hospital evaluations. Please keep in mind though, that different hospitals are different per unit, and what might not be a perfect fit for one nurse could be a great fit for another nurse.

There only two things that I ask of you… Please keep in mind that this is NOT a bashing form, all negative reviews will be investigated before being made live on the site. Most of all, be honest with yourself, the hospital, and other traveling nurses.

Was the hospital clean and well maintained?

Was there dedicated staff parking?

Was there a fee for parking?

Was parking secure with security/cameras?

Was there a cafeteria with hot meals for employees for dayshift and nightshift?

Were there in-house coffee shops or other food vendors available?

Was signage in the facility adequate for finding the directions to different units?

Questions related to interview/unit assigned

Was this an auto offer?

Did you receive an interview with the unit manager/director?

Did they represent the unit/facility appropriately and honestly?

Did you feel that the unit was “traveler” friendly?

Were the patient ratios appropriate? 1:5 MS, 1:4 Tele, 1:3 PCU/SD/IMC, 1:4 ER, 1:2 ICU, 1:2 L&D, 1:4 couplets PP)

Did you feel that you received adequate orientation for the unit?

If not, what amount of time did you get or why was it unneccesary?

Did you receive hospital orientation as well?

How long was this orientation?

Did you feel that your first day instructions provided you with the necessary information needed?

Did you actually meet the unit/interviewing manager prior to starting work?

Did you have designated rounds, handoff reports etc.?

Questions About the Hospital Culture

Was this a teaching hospital?

If yes, did you work directly with the residents?

Did the physicians treat the nurses with respect and were they available when necessary?

Was the cohesion and collaboration with all departments such as nursing, PT, OT, RT, Lab, etc.?

How did you maintain communication within the facility? IE vocera, call lights, etc.

Was the communication between caregivers and staff effective?

Was equipment available within a reasonable amount of time?

Were medications and supplies readily available?

Were there lifts to help with patient mobility?

Was isolation used correctly and effectively?

Was patient safety a priority?

Was the staff professional and friendly to travelers?

Were there assistants (CNAs & techs) available?

If they were available, were they willing to help?

Did you have self-scheduling?

Did they honor your request for the most part?

Did they allow blocked scheduling?

Did they allow 6 to 8 days straight?

Did you have a specific color of uniform or uniform “rules”? ( ie. No t-shirts, etc.)

Were you required to be on call?

Did you have a set requirement of weekend shifts?
